Community Hub

In an effort to make improvements to how we coordinate our care, we have introduced a new standalone phone system with one phone number –
02382 548860 – for our community palliative care team, who can offer advice and information to patients and their families, as well as to health and social care colleagues.

Calls to this number will come through to our Community Hub, which is manned by two Clinical Nurse Specialists, an Administrator and Community Support Worker from 8:30am to 4:30pm, seven days a week. 

Out-of-hours calls for urgent advice will continue to be answered by our team on the InPatient Unit. You will also be able to leave any non-urgent messages for our team, which will be picked up the following day.
